Athletic Permission Form 26-27- Updated

This permission form must be completed for students to participate in after school athletics. Please select which sport your child(ren) will be participating in each season. If they're not 100% sure on a sport, it's okay to select it for now. They will have permission to select a different sport or change their mind later in the school year.
