Click on the image below to join our athletics group and fill out the permission form for Cole Athletics. This permission form will cover athletes for the clinic and tryouts. Athletes will only be able to attend if they have this form completed! If your athlete has already participated in a sport this year, the QR code will say "You are already a member of this group" This means that your athlete is clear to participate in the clinic and tryouts. This permission slip covers athletics for the 25-26 school year. Participating in athletics including cheer next school year will require a new form to be filled out at the start of the new school year.
